Please fill in all details. Save a copy for your records then email to admin@nurse.org.nz
Payments can be made by Cheque, Direct Debit or Credit Card.
- Cheque payments - please send registration form together with your Cheque in the post.
- Direct Credit payments - please arrange this first as per the instructions on the form then include exact details of your payment on the Registration form. We must be able to identify your Direct Debit payment before we can register you for the Symposium.
- Credit Card Payments – please fill in all Credit card details on the form or phone the College office (06) 358 6000 with your details to make a Credit Card Payment. Please note – We are pleased to be able to offer this facility for your convenience but there is an additional $10 charge for this service. This facility has been set up on customer request, unfortunately due to the extra bank fees incurred we are unable to accept Credit card payments without this additional fee.
